Overview
Factor XI inhibition offers a unique mechanism for anticoagulation by inhibiting the intrinsic pathway of the coagulation cascade while attenuating hemostasis via the extrinsic pathway. This mechanism provides a potential advantage over traditional anticoagulation by reducing clot risk while maintaining a low bleed risk. The clinical trial data is heavily mixed depending on the indication for use. Nonetheless, the first positive phase III trial involving a Factor XI inhibitor was published in early 2026. These reasons ultimately highlight the need to discuss these upcoming agents to prepare pharmacists for appropriate and safe utilizations as they approach market approval.

Objectives
Upon completion of this activity, participants will be able to:
1. Review the background surrounding history of Factor XI inhibitors and their impact on the clotting cascade
2. Evaluate the clinical trials available to date for proposed Factor XI inhibitor use for different indications
3. Discuss clinical considerations regarding future use of Factor XI inhibitors
