Overview
Butyrylcholinesterase (BCHE) deficiency is an important yet underrecognized pharmacogenomic condition that can result in prolonged neuromuscular blockade following succinylcholine administration, creating significant implications for patient safety in surgical and emergency care settings. As St. Elizabeth implements an in-house pharmacogenomic panel that includes BCHE and incorporates related clinical decision support tools; pharmacists will play an increasingly important role in interpreting results and guiding medication selection. This presentation will review the genetics, diagnosis, and clinical management of BCHE deficiency while highlighting practical strategies pharmacists can use to identify at-risk patients and optimize care when prolonged paralysis occurs.

Objectives
Upon completion of this activity, participants will be able to:
1. Explain the pharmacology of succinylcholine and neuromuscular blockade monitoring.
2. Describe the role of BCHE in the metabolism of succinylcholine and other ester-containing medications
3. Apply pharmacogenomic and clinical evidence to optimize anesthetic medication selection in patients with known or suspected BCHE deficiency
4. Recognize common inherited and acquired causes of BCHE deficiency and their effect on neuromuscular blockade duration
