Overview
CMV remains one of the most common and clinically impactful opportunistic infections in solid organ transplant recipients, contributing to increased graft dysfunction and mortality. Despite established prophylaxis and first-line therapies, it is not uncommon to encounter treatment-refractory or antiviral-resistant CMV disease. Refractory disease and resistance pose treatment challenges. This presentation addresses a need to enhance understanding of definitions, resistance testing, and evolving treatment strategies to support evidence-based management and optimize transplant outcomes.

Objectives
Upon completion of this activity, participants will be able to:
1. Describe CMV in solid organ transplant recipients including standard prophylaxis, risk factors for development, and patient presentation  
2. Differentiate treatment-refractory and antiviral-resistant CMV disease
3. Identify indications for resistance testing for CMV infection
4. Apply guidelines-based and alternative therapies to manage refractory and/or resistant CMV infection
