Overview
Myasthenia gravis is a complex autoimmune disorder characterized by autoantibodies that disrupt neuromuscular transmission, leading to fluctuating muscle weakness and potentially life-threatening respiratory failure known as myasthenic crisis. Managing these acute exacerbations requires care teams to quickly identify triggers, provide specialized supportive care, and navigate the nuances of therapies like IVIG and PLEX.
The treatment landscape for refractory myasthenia gravis has undergone a shift with the rapid emergence and FDA approval of novel biologic therapies. This grand round is designed to bridge the gap between traditional management and these emerging therapies, emphasizing the pharmacist's critical role in both acute crisis management as well as optimizing dosing, ensuring medication safety, and coordinating the complex transition of care for patients with refractory disease.
Objectives
Upon completion of this activity, participants will be able to:
1. Discuss the pathophysiology of myasthenia gravis and its effect on neuromuscular transmission
2. Review strategies for myasthenia gravis crisis management
3. Evaluate biologic therapies used in refractory myasthenia gravis by comparison of mechanism of action, landmark trials, and clinical pearls
