Overview
Acute liver failure (ALF) presents a unique and often misunderstood hemostatic profile in which patients are simultaneously at risk for both bleeding and thrombosis despite markedly abnormal laboratory values such as elevated INR. Traditional markers of coagulopathy do not accurately reflect bleeding risk, frequently leading to inappropriate transfusions or hesitation to initiate necessary anticoagulation. This creates significant variability in clinical practice and potential harm, highlighting the need for improved understanding of hemostasis in ALF and evidence-based approaches to managing both bleeding and thrombotic complications.
Objectives
Upon completion of this activity, participants will be able to:
1. Recognize coagulopathy differences in Acute Liver Failure (ALF), Acute-on-Chronic Liver Failure (ACLF), and cirrhosis.
2. Evaluate the utility of various laboratory parameters in assessing the degree of coagulopathy.
3. Identify when coagulopathy reversal is necessary and which treatment options are safe and effective.
4. Discuss the risks and benefits of anticoagulation in patients with ALF.
