Overview
For patients undergoing CRRT, anticoagulation is an essential aspect of care in order to maintain circuit patency and function. Traditional options for anticoagulation consist of heparin and citrate. However, these therapies present challenges in patients with elevated bleeding risk or organ dysfunction. Epoprostenol, a prostacyclin analog, has emerged as a potential alternative anticoagulant for patients undergoing CRRT. This presentation will review current recommendations, explore the role and mechanism of epoprostenol, and compare available strategies for anticoagulation in patients undergoing CRRT. 

SGLT2 inhibitors are emerging as a therapeutic option for pediatric CKD, a field where current treatments are extrapolated from adult data. Recent pediatric case series and small clinical trials have supported the use of SGLT2 inhibitors for pediatric CKD. Importantly, the results of the ongoing DOUBLE PRO-TECT Alport trial are highly anticipated, as they represent a significant step in assessing the role of SGLT2 inhibitors in pediatric kidney protection. This presentation will provide the most up to date information on clinical evidence for use of SGLT2i in pediatric CKD.

 

Objectives
Upon completion of this activity, the participant will be able to:
1. Describe current recommendations regarding anticoagulation for patients undergoing CRRT
2. Recognize how epoprostenol would work as an anticoagulant in CRRT
3. Compare/contrast current options for anticoagulation in CRRT
4. Describe the mechanism of action, safety profile, and potential adverse effects of sodium-glucose cotransporter 2 (SGLT2) inhibitors in pediatric Chronic Kidney Disease (CKD). 
5. Evaluate current evidence for the use of SGLT2 inhibitors in pediatric CKD. 
6. Formulate evidence-based recommendations for the integration of SGLT2 inhibitors into pediatric COD management.
