Denise Fabian, MD

I am an Associate Professor of Radiation Oncology and Residency Program Director at the University of Kentucky Markey Cancer Center, where I also serve as Director of Gynecologic Radiation Oncology and Brachytherapy. I lead a high-volume, tertiary referral gynecologic brachytherapy program that provides comprehensive care for patients with cervical and endometrial cancers, including those with locally advanced, recurrent, and high-risk disease. Under my leadership, the program has become one of the most active and technically advanced in the region, integrating intracavitary and interstitial brachytherapy and novel radiation techniques to improve oncologic outcomes and organ preservation. My research program focuses on clinical and translational investigation of advanced radiation strategies and radiopharmaceutical therapies for gynecologic malignancies, with a particular emphasis on cervical cancer populations in Appalachia. I am the principal investigator on multiple investigator-initiated Phase 0, Phase I, and Phase II clinical trials evaluating hypofractionated radiochemotherapy, cesium-131 low-dose-rate interstitial brachytherapy, spatially fractionated radiotherapy, and alpha-particle theranostic agents targeting fibroblast activation protein (FAP). I am also an active member of NRG Oncology and serve on the Cervix/Vulvar, Radiation Oncology, and Theranostics Committees, contributing to national cooperative group trial development. My work is supported by institutional funding and has resulted in numerous peer-reviewed publications in high-impact journals, including Gynecologic Oncology; Annals of Oncology; International Journal of Radiation Oncology, Biology, and Physics; and Frontiers in Oncology. In parallel with my research efforts, I am deeply committed to physician-scientist training and mentorship. As Residency Program Director, I oversee a comprehensive educational curriculum that emphasizes clinical excellence, research literacy, and professional development. Collectively, my clinical expertise, leadership roles, and research focus position me well to contribute meaningfully to NIH-funded investigations aimed at improving outcomes and survivorship for women with gynecologic cancers.
