Pediatric Thrombosis

Pediatric thrombosis, while relatively rare, remains a clinically significant issue that is increasingly encountered, particularly among hospitalized children. Despite its importance, there is a recognized gap in clinicians’ knowledge and confidence regarding the evaluation and management of thrombotic events in children. Many pediatricians are unfamiliar with established approaches to thrombophilia evaluation and the interpretation of test results. Additionally, there is limited understanding of the appropriate selection, dosing, and monitoring of commonly used anticoagulants in both inpatient and outpatient pediatric settings. Uncertainty also persists regarding evidence-based recommendations for the duration of anticoagulant therapy in children with venous thromboembolism (VTE). These gaps highlight the need for comprehensive education to improve pediatric providers’ competency in the assessment and management of pediatric thrombosis and to enhance patient safety and outcomes.

Learning Objectives

Upon completion of this module, participants will be able to:

  • Explain the pathophysiological mechanisms underlying pediatric venous thromboembolism (VTE) to inform diagnostic reasoning and management decisions.
  • Analyze the contribution of genetic and acquired risk factors to VTE development in pediatric patients to guide targeted investigations.
  • Compare pharmacologic and non-pharmacologic treatment strategies for pediatric VTE, emphasizing mechanisms of action, monitoring, and potential complications.
  • Evaluate how clinical variables such as age, comorbidities, and provoking factors influence the duration and intensity of VTE therapy in children.

Dr. Smith is a board certified pediatric hematologist and oncologist at UK Healthcare. He completed his residency training in pediatrics at the University of Tennessee Health Science Center in Memphis, TN, followed by fellowship at Baylor College of Medicine in Houston, TX. He is clinical faculty taking care of a wide variety of hematologic and oncologic conditions, but has interest in bleeding and clotting disorders, heavy menstrual bleeding in adolescents, and iron deficiency anemia
