Implementing Team-Based Care Models in Rural Areas: How Nurse-Midwives and Nurse Practitioners Can Help Improve Outcomes

Team-based health care is the provision of health services to individuals, families, and/or their communities by at least two health providers who work collaboratively with patients and their caregivers—to the extent preferred by each patient—to accomplish shared goals within and across settings to achieve coordinated, high-quality care.

Target Audience

This course targets physicians, midwives, advanced nurse practitioners, nurses, community health workers, peer support specialists, social workers, doulas, emergency medical technicians, pharmacists, public health officials, and individuals with lived experience.

Learning Objectives

Upon completion of this activity, participants will be able to:
1. Define and explain the team-based health care framework's relevance to improving maternal health in rural communities.
2. Describe how decentralized, nurse-led models—such as the integration of midwifery, primary care, and mental health services address gaps in maternity care access and outcomes across rural Kentucky.
3. Identify key elements that support successful implementation of collaborative care models.
