Extension 101 for Healthcare

Kentucky continues to experience substantial population health challenges, including a high burden of chronic disease and tobacco use. Improving these outcomes requires attention not only to clinical care but also to the social and community factors that influence health behaviors, disease prevention, and chronic disease self-management. Healthcare professionals across disciplines are well positioned to connect patients with community-based resources; however, a knowledge gap exists regarding the role of Cooperative Extension and the free or low-cost health education and prevention programs available through its offices across Kentucky.

An educational need therefore exists to increase healthcare professionals’ knowledge of Cooperative Extension, its mission as part of the Land-Grant University system, the health-related programs and services available within Kentucky communities, and its potential role as a trusted resource for patient education and support. Addressing this gap can better equip healthcare professionals to connect patients with accessible community-based education that supports disease prevention, health promotion, and chronic disease self-management.

Nurses, Community Health Workers, Social Workers, Dietitians, Nurse Practitioners, Physician Assistants, and Physicians

Learning Objectives

Upon completion of this module, participants will be able to:

  • Describe the role of Cooperative Extension within the Land-Grant University system in supporting community health education.
  • Identify free or low-cost Cooperative Extension health education resources available to patients across Kentucky.
  • Explain how community-based health education resources relate to patient health behaviors.
  • Select appropriate Cooperative Extension resources for patient health education needs.
