Error Prevention Workshop

Kentucky Children's Hospital and the Department of Pediatrics are striving to build in a culture of safety that prioritizes just culture concepts. Our Culture of Safety Index for staff and providers is at or below national benchmarks when compared nationally to other Children's Hospitals. In addition, Kentucky Children's seeks to improve Safety and Quality and be a top performer on national databases like US News and World Report, Solutions for Patient Safety, Vermont Oxford Network (NICU), and Vizient. Many of the principles taught here will help support an environment of error reporting and learning that will build upon our performance improvement and harm reduction work.

Learning Objectives

Upon completion of this activity, participants will be able to:
1. Describe the principles of the safety culture and its reliance upon just culture, error reporting, and a learning environment​
2. Discuss Modern Safety Principles, including Proactive Safety Techniques
3. Discuss human error modes and how high reliability tactics to prevent them
4. Discuss the conditions that lead to serious patient and employee safety events
5. Describe and practice error [prevention tactics ion everyday clinical situations​
