UK HealthCare recognizes the importance of an established safety culture where modern safety principles are employed. These principles include the use of proactive safety techniques such as simulation, safety huddles, walk-through/talk-through, and learning teams. Identification of human error modes (e.g., knowledge-based performance errors, rule-based performance errors, and skill-based performance errors) and how to mitigate them using the high reliability tactics of STAR (Stop, Think, Act, and Review), SBAR (Situation, Background, Assessment, and Recommendation). Team Member Checking, and using clarifying questions.

Dr. Tariq Chaudry, MD, FAAP, is a results-driven medical provider at UK HealthCare, with a 22-year history of specializing in the care of the pediatric patient. He holds board certifications in General Pediatrics and Pediatric Hospital Medicine and currently serves as a Pediatric Hospitalist in the Kentucky Children’s Hospital and Associate Chief Quality Officer. Dr. Chaudry’s academic appointments include Assistant Professor in the Department of Pediatrics, Division of Hospital Medicine at the University of Kentucky College of Medicine, and Vice Chair of Quality, Value, and Safety within the Department of Pediatrics, Division of Hospital Medicine.
