Overview
The rising prevalence of hospitalized patients with moderate to severe ulcerative colitis and Crohn’s disease has emphasized the role of pharmacists in the evaluation and management of inflammatory bowel disease (IBD) in the inpatient setting. Pharmacists must be able to identify clinical presentations, review treatment history, and guide therapeutic decisions, including the continuation of home immunologic agents or the initiation of inpatient biologics or small molecule therapies. This requires familiarity with guideline-directed step therapy and institution-specific formulary utilization.

This continuing education session will provide pharmacists with a comprehensive review of inpatient IBD management, including disease assessment, evidence-based treatment strategies, therapeutic monitoring, transitions of care, and formulary navigation. Attendees will be better prepared to make informed clinical recommendations, improve access to appropriate therapies, and advocate for formulary changes when supported by patient care needs and guideline-directed recommendations.

Objectives
Upon completion of this activity, the participant will be able to:
1. Analyze the pathophysiologic and clinical features of ulcerative colitis and Crohn’s disease to support inpatient decision-making
2. Evaluate guideline- and primary literature-directed stepwise treatment strategies for hospitalized patients with ulcerative colitis and Crohn’s disease
3. Apply institutional protocols, formulary criteria, and monitoring parameters to optimize the inpatient use of immunologic agents for ulcerative colitis and Crohn’s disease
