Connecting to Commonwealth: A Lung Transplant Conversation is a specialty-focused educational program that brings together community pulmonologists, advanced practice providers, and other healthcare professionals with lung transplant specialists from UK HealthCare to discuss contemporary approaches to the care of patients with advanced lung disease. Through interactive presentations and case-based discussion, participants will review current evidence, referral considerations, patient evaluation, pre-transplant optimization, and long-term post-transplant management.

Designed to foster collaboration across the continuum of pulmonary care, this program emphasizes practical strategies that support timely referral, multidisciplinary communication, and coordinated management of patients with complex pulmonary conditions. Participants will leave with actionable knowledge that can be readily applied within their clinical practice to improve patient access, care coordination, and outcomes.

Upon completion of this activity, participants will be able to: 
1. Identify patients with advanced lung disease who may benefit from referral to a specialty pulmonary or lung transplant center. 
2. Determine the optimal timing for referral based on current evidence, disease progression, and established referral criteria. 
3. Apply evidence-based strategies to optimize patients prior to specialty evaluation and lung transplantation, including management of comorbid conditions and appropriate medical therapies. 
4. Describe current approaches to post-transplant co-management, including immunosuppressive therapy, infection prevention, long-term surveillance, and coordination of care between transplant specialists and community providers.
5. Demonstrate improved collaboration and communication with the UK HealthCare Lung Transplant Program to enhance continuity of care and improve outcomes for patients with advanced lung disease.
