Overview
COPD remains a leading cause of morbidity, mortality, and healthcare utilization, with many patients continuing to experience exacerbations despite optimized inhaled therapy. As our understanding of COPD pathophysiology has advanced, management has shifted from a traditional inhaler-focused approach toward phenotype-driven, precision medicine strategies. Emerging evidence has identified an eosinophilic/type 2 inflammatory COPD phenotype that may respond to targeted biologic therapy, offering a new treatment option for patients with persistent disease burden. This topic is especially timely given the 2025 GOLD guideline inclusion of dupilumab as the first biologic therapy recommended for select patients with COPD and elevated blood eosinophil counts. Landmark trials have demonstrated reductions in exacerbations and improvements in lung function with biologic treatment, highlighting a potentially practice-changing advancement in COPD care. Despite these developments, many clinicians may be unfamiliar with interpreting eosinophil counts or identifying appropriate candidates for biologic therapy. This presentation will review recent updates in COPD management, discuss the rationale for phenotype-based treatment selection, and evaluate the evidence supporting biologic therapies. It will also address practical considerations for implementing these therapies in clinical practice and highlight the important role of ambulatory care, primary care, and specialty pharmacists in optimizing patient outcomes.
