Overview
Acute ischemic stroke (AIS) is a leading cause of morbidity and mortality worldwide, and timely reperfusion therapy is critical to improving outcomes. Alteplase has historically been the agent of choice for intravenous (IV) thrombolysis. However, tenecteplase has emerged as a potential alternative. Recent clinical trials have demonstrated that tenecteplase is at least non-inferior to alteplase in terms of functional outcomes and safety, with some studies suggesting superior efficacy. Additionally, tenecteplase may offer logistical benefits in emergency and pre-hospital settings, potentially reducing door-to-needle times and improving workflow efficiency. Despite this, tenecteplase use varies across institutions. This presentation aims to provide an evidence-based overview of tenecteplase in AIS, highlighting key trials, guideline recommendations, and practical considerations for its implementation into clinical practice.

Objectives
Upon completion of this activity, the participant will be able to:
1. Review the pharmacologic and clinical differences between alteplase and tenecteplase 
2. Discuss current evidence for tenecteplase use in acute ischemic stroke 
3. Identify appropriate dosing strategies and safety considerations for tenecteplase use
