Overview
Ensuring that patients remain up to date with recommended vaccinations is a shared, interdisciplinary responsibility that continues to evolve as new evidence emerges. This responsibility is particularly critical for immunocompromised individuals, who face an increased risk of vaccine-preventable infections and related complications. Vaccine schedules differ for immunocompromised patients compared to healthy patients, and these differences can add an additional layer of complexity for healthcare workers in determining eligibility for immunization. In 2025, the Infectious Diseases Society of America released updated guidelines outlining immunization recommendations for respiratory viruses in immunocompromised patient populations. While UofL Health has established screening and administration policies for routine vaccines such as pneumococcal and influenza vaccines, institution-specific guidance tailored to immunocompromised populations is currently limited.

Patients receiving care through the Brown Cancer Center and the Trager Transplant Center represent a population in which pharmacists are uniquely positioned to optimize immunization practices. This educational session will review updated vaccination recommendations for immunocompromised patients and highlight practical clinical resources to support guideline-directed, evidence-based immunization decision-making within UofL Health.

Objectives
Upon completion of this activity, the participant will be able to:
1. Identify current clinical guidelines and evidence-based resources guiding vaccination practices in immunocompromised patient populations
2. Describe updates to vaccination recommendations for immunocompromised patients, including those with malignancies, solid organ transplants, and other high-risk conditions such as asplenia
3. Evaluate vaccination history in immunocompromised patients
4. Recommend appropriate additional or catch-up immunizations
