Overview
Sleep disorders, particularly chronic insomnia, affect approximately 10-15% of adults and are frequently encountered in ambulatory care settings, yet the use of traditional pharmacotherapy options has been limited by concerns over dependence, tolerance, and cognitive impairment. The emergence of new medications, primarily dual orexin receptor antagonists (DORAs), represents a mechanistically distinct approach to treating insomnia, targeting the wake-promoting orexin system rather than broadly enhancing inhibitory pathways. With three FDA-approved agents now available (suvorexant, lemborexant, and daridorexant), pharmacists are uniquely positioned to guide appropriate selection, counsel patients on expectations, and optimize therapy. This presentation will equip clinicians, including physicians and pharmacists, with the knowledge to confidently integrate these novel agents into practice.

Objectives
Upon completion of this activity, participants will be able to:
1. Describe the pathophysiology of insomnia and the mechanisms involved in sleep-wake regulation. 
2. Compare non-pharmacological and pharmacological treatment options for insomnia, including traditional agents and dual orexin receptor antagonists (DORAs). 
3. Identify appropriate patient candidates for DORA therapy based on clinical characteristics, comorbidities, and concomitant medications. 
4. Discuss opportunities for pharmacist involvement in the management of insomnia.
