Overview
Pulmonary hypertension is a complex disease state requiring multidisciplinary management with high-risk medications. Our advanced heart failure providers are beginning to care for more of these patients, and therefore we will begin to encounter these medications more frequently in the hospital. Our policies and procedures surrounding parenteral prostacyclin therapies have recently been updated, and our heart failure pharmacists will provide a brief review of pulmonary hypertension management and education regarding the updates and expectations of pharmacists.
Objectives
Upon completion of this activity, participants will be able to:
1. Review etiology, pathophysiology, and diagnosis of PH/PAH
2. Discuss medication classes used to treat PAH and their mechanism of action, dosing strategies, side effect profiles, and clinical pearls
3. Outline the updated Parenteral Prostacyclin Policy and its appendices
4. Describe the pharmacist responsibilities when caring for this patient population at SEH
