Overview
Pulmonary embolism remains one of the most clinically challenging and critical conditions encountered in acute care. Despite its frequency, practice patterns remain different throughout healthcare systems, particularly in areas such as risk classification and use of therapeutic monitoring. The recently published 2026 AHA/ACC Acute Pulmonary Embolism Guideline represents the first comprehensive, multi-society effort to standardize the evaluation and management of acute PE. The new guidelines introduce a clinical classification system (Categories A–E) that shifts the approach from traditional “massive vs submassive” to a risk-based system. This change is particularly impactful because it directly links patient presentation to management decisions, including level of care, need for advanced interventions, and appropriateness of outpatient treatment. Given the complexity of these decisions, it is essential for pharmacists and physicians to understand not only what the guideline recommends, but also how to apply it in for individualized patient care. This presentation will effectively educate professionals by translating guideline updates into practical, case-based decision-making strategies, with a focus on risk stratification, treatment selection, and management of special populations. Ultimately, this presentation will support consistent, evidence-based care and better alignment between emerging literature and pragmatic practice.

Objectives
Upon completion of this activity, participants will be able to:
1. Review the epidemiology, pathophysiology, and risk factors of pulmonary embolism
2. Discuss updates in the new AHA/ACC 2026 PE guidelines vs. historic PE guidelines
3. Evaluate appropriate diagnostic strategies for PE based on new guideline criteria
4. Develop treatment plans for PE based on risk category
5. Apply guideline-directed management strategies for PE in special populations
