Need/Overview
Hepatic Arterial Infusion (HAI) represents a specialized regional chemotherapy delivery method, utilizing a surgically implanted pump to administer chemotherapeutic agents directly to liver tumors. HAI has multiple indications for GI malignancies and is most frequently utilized in, metastatic colorectal cancer without extrahepatic involvement and intrahepatic cholangiocarcinoma.
HAI therapy stems from its unique ability to deliver high concentrations of chemotherapy directly to hepatic tumors, thereby maximizing local tumor exposure while significantly minimizing systemic toxicity. Given the expanding utilization of HAI in advanced cancer care, a comprehensive understanding of its mechanism of action, clinical applications, and potential toxicities is imperative for pharmacists. This specialized knowledge is crucial for optimizing patient outcomes and ensuring the provision of safe, evidence-based pharmaceutical care within the oncology setting.
Objectives
Upon completion of this activity, participants will be able to:
1. Define Hepatic Arterial Infusion (HAI) and its role in oncology.
2. Review the rationale and complex dosing strategies for the common HAI chemotherapeutic agent, floxuridine.
3. Discuss HAI place in treatment guidelines
4. Assess common HAI toxicities and the pharmacist's role in their monitoring and management.
