Needs/Overview
Cangrelor is a reversible, parenteral P2Y12 inhibitor with a rapid onset and short half-life. It is currently indicated as an adjunct to PCI for patients who have not been treated with an oral P2Y12 inhibitor and are not planned to receive a glycoprotein IIB/IIIa inhibitor. In most cases, patients can be loaded with an oral P2Y12 inhibitor, but for patients who have altered absorption cangrelor could be an option for platelet inhibition. The 2021 ACC/AHA/SCAI Guideline for Coronary Artery Revascularization states that intravenous cangrelor may be reasonable to reduce periprocedural ischemic events. With more studies coming out comparing cangrelor to other agents in the periprocedural setting, it is important to both understand when and how cangrelor can be used, and the evidence that supports its use. This presentation will review guidelines, as well as an overview of trials that provide insight on cangrelor’s place in therapy.
Objectives
Upon completion of this activity, participants will be able to:
1. Describe the pharmacokinetics and pharmacodynamics of cangrelor in comparison to oral P2Y12 inhibitors
2. Review cangrelor's current FDA indication and place in therapy based on guideline recommendations
3. Identify patient characteristics and scenarios that may be appropriate for cangrelor use
