Need/Overview
Direct oral anticoagulants (DOACs) have revolutionized anticoagulation therapy by offering simplified dosing and fewer monitoring requirements compared to traditional agents like warfarin. However, their use in special populations—such as, but not limited to, patients with obesity, renal or hepatic impairment, or those post-bariatric/gastric surgery—presents unique clinical challenges. These groups are often underrepresented in pivotal trials, leaving gaps in evidence-based guidance. Staying current on emerging data and guideline updates is essential to ensure safe and effective anticoagulation management. This presentation will provide a focused review of recent literature, clinical pearls, and practical strategies for optimizing DOAC therapy in complex populations. By enhancing understanding of pharmacokinetic considerations, dosing adjustments, and risk mitigation, practitioners will be better equipped to make informed decisions and improve patient outcomes in real-world settings.
Objectives
Upon completion of this activity, participants will be able to:
