Needs/Overview
Euglycemic diabetic ketoacidosis (EDKA) is a rare but serious complication of diabetes mellitus, occurring in both type 1 and type 2 patients. It is characterized by metabolic acidosis (arterial pH <7.3, serum bicarbonate <18 mEq/L), elevated anion gap, and ketonemia, despite a normal glucose level (<200 mg/dL). Risk factors include prolonged fasting, surgery, pregnancy, infection, and the use of SGLT2 inhibitors, which are increasingly suspected in EDKA cases. Additionally, recent studies have suggested other potential medication causes such as glucagon-like peptide 1 agonists (GLP-1s). Due to its varied etiologies and the lack of marked hyperglycemia, EDKA is difficult to recognize and poorly understood, often delaying diagnosis. Timely recognition and treatment are essential, and pharmacists can play a critical role in identifying at-risk patients while supporting clinical decision making. This presentation will assist in educating pharmacists and providers on current best practices for recognizing and managing EDKA.
Objectives
Upon completion of this activity, participants will be able to:
1. Define euglycemic diabetic ketoacidosis (EDKA) and review its pathophysiology
2. Recognize the clinical presentation and appropriately diagnose EDKA
3. Identify risk factors and medication-related causes of EDKA
4. Discuss treatment strategies and the pharmacist’s role in managing EDKA
